Imprima y firme el formulario de solicitud.<\/span><\/p><p><span style=\"font-weight: 400;\">Link: https:\/\/app.iess.gob.ec\/iess-gestion-solicitud-afiliado-web\/public\/pagina.jsf.<\/span><\/p><p><span style=\"font-weight: 400;\">&#8211; Ingrese al portal www.iess.gob.ec, escoja la opci\u00f3n \u2018Tr\u00e1mites virtuales\u2019, seleccione el \u00edcono \u2018Ventanilla virtual\u2019, a continuaci\u00f3n, se desplegar\u00e1 el formulario de \u2018Recepci\u00f3n de tr\u00e1mites\u2019, ingrese sus datos correctamente y elija la provincia, tr\u00e1mite, detalle su pedido en el mensaje y anexe todos los requisitos en formato JPG o PDF (peso m\u00e1ximo 1MB).<\/span><\/p><p><span
